INTRODUCTION
DOT - Recordable Accident Register Report is a report used to record a brief summary of all reportable accidents and incidents that have occurred annually.
A reportable accident or incident is one in which a vehicle was towed from the scene, or an injury or fatality occurred.
This information must be maintained for three years and made available to any FMCSA agent, authorized state or local enforcement agency representative, or authorized third party.
Each establishment appears on a separate page of the report.
INJURY OR FATALITY
A motor vehicle accident involving an injury or fatality under the following will appear on the report under the following circumstances:
- Incident Type: Must be both Injury and Motor Vehicle
- Severity: Fatality AND/OR Medical Treatment
- Asset: Has one or more of the following:
- GVWR or GCWR greater than 10,000 pounds
- Seating capacity more than 9 people
- Asset was displaying a hazardous materials placard
Follow these steps to complete an incident that includes an injury or fatality:
- Search, locate and edit the incident.
- Incident Tab: Refer to the Incident Tab article for instructions to complete this section.
- Incident Type: Check both Injury and Motor Vehicle.
- Motor Vehicle Incident Details: Scroll down the page and complete this section. Refer to the Motor Vehicles Incident Details section of the Incident Tab article to learn how to complete this section.
- Injuries Tab: Severity must be Fatality OR Medical Treatment. Refer to the Injuries Tab article to complete this section.
- Asset Tab: Refer to the Assets Tab article for instructions to complete this section.
Note: Before you can add an asset to the incident, the asset must be added to the Incident Management System (IMS). Please refer to the Assets: Overview - learn how assets are used by the IMS. The GVWR, GCWR and seating capacity is added when creating an asset. If you do not follow the steps to create the asset, the incident will not appear on the Recordable Accident Register Report. - Follow the steps below to run the DOT Recordable Accident Register Report.
TOWED FROM THE SCENE
A motor vehicle accident that is towed from the scene will appear on the report under the following circumstances:
- Incident Type: Must be Motor Vehicle
- Asset: Has one or more of the following:
- GVWR or GCWR greater than 10,000 pounds
- Seating capacity more than 9 people
- Asset was displaying a hazardous materials placard
- Towed set to YES
Follow these steps to complete an incident that is towed from the scene:
- Search, locate and edit the incident.

- Incident Type: Check Motor Vehicle.
- Motor Vehicle Incident Details: Scroll down the page and complete this section. Refer to the Motor Vehicles Incident Details section of the Incident Tab article to learn how to complete this section.
- Asset Tab: Refer to the Assets Tab article for instructions to complete this section.

RUN AND PRINT THE REPORT
Follow these steps to the DOT - Recordable Accident Register Report
- Click the Administration tab at the top of the screen.
- Click Incident Management, then Reports on the left menu bar.
- Select the View button (Eye icon) located to the left of the DOT-Recordable Accident Report.
- Establishment: Select on or more establishments as follows:
- Click the Select button to the right of the Establishments field.
- Click the checkbox to the left of the establishment name and use the right arrow to move to the Selected Records table.
- Repeat this process until all establishments are added to the Selected Records table.
- Click the Select button in the upper left corner of the screen.
- Start Date: Click the Calendar icon to select the start date.
- End Date: Click the Calendar icon to select the end date.
- Click the View Report button to view the report.
- Refer to the Reports: Run, Print, Export and Email article for instructions to print, export and email the report.
