INTRODUCTION
The Injuries Tab allows you to create entries for each injury that was a result of the incident. A single entry can contain multiple injuries for a single user or contact.
The Injuries Tab is only completed when injury and/or illness is selected as the incident type.
ADD AN INJURY
These instructions assume you have already added the users and contacts to the incident. Please refer to the Add Users and Contacts article to learn how to add users and contacts to an incident.
Follow these steps to add an injury to the Injuries tab. Required entry fields are denoted in RED. Optional entry fields are denoted in BLACK.
- Search and locate the desired incident.
- Click on the Injuries tab.
- Click the Add an Injury button.
Note: The Injuries screen always includes Details, Body Parts and OSHA Compliance sections. The screen will present additional fields or sections based on the choice made in severity. Click on the links below to learn how to complete each section:

Details
- Injured Person: Select the injured person from the drop down list.
Note: If you do not see the person you would like to select, you have not added them to the People tab. Please refer to the Add Users and Contacts article to learn how to add users and contacts to an incident. - Severity: Check any severity that applies. New sections or fields will be added based on severity.
- Date of Death: Enter the date of death.
Note: This field is only added when you select Fatality as a severity. - Time Work Began: Enter the time the work began.
- What was the Injury/Illness: Type the injury or illness.
- What Object or Substance Directly Harmed the Employee: Type the details that harmed the employee.
- Privacy Case: Check to mask the name of the injured employee on OSHA Form 300.
- Take the following actions:

Timeline
The Timeline section will appear when Lost Workdays and/or Restricted Workdays and Transfer are selected as a severity
Follow these steps to complete the Timeline Section. Required entry fields are denoted in RED. Optional entry fields are denoted in BLACK.
Lost Workdays: Complete the following fields:
- Days Away from Work: Type the number of days away from work.
- Away Start Date: Select the start date.
- Days Restricted Description: Type an applicable description.
- Date Returned: Select the Date Returned.
Restricted Workdays and Transfers: Complete the following fields:
- Days Restricted: Type the number of days restricted.
- Restricted Start: Select the start date.
- Days Restricted Description: Type an applicable description.
- Date Returned: Select the Date Returned.
Take the following actions:

Treatment Facilities
The Treatment Facilities section will appear when Medical Treatment is selected as a severity.
Notes: Treatment Facilities must be added to the system prior to adding them to the incident. If you are unable to locate the treatment facility, refer to the Treatment Facilities: Add, Edit and Delete article to learn how to add a treatment facility.
Follow these steps to complete the Treatment Facilities section. All entry fields are denoted are optional entry fields and are denoted in BLACK.
- Treatment Facility: Follow these steps to select a Treatment Facility:
- Click the Select a treatment facility link or the Select button.
- Search for the treatment facility in the search field or scroll down the list to find the treatment facility.
- Click the checkbox to the left of the desired treatment facility.
- Click the Select button in the upper left corner of the screen.
- You will be returned to the Treatment Facilities section of the Injuries tab.
- Treated on: Select the date the user was treated.
- Provider Name: Type the provider (treating physician's) name.
- Treatment Description: Type an applicable description.
- Emergency Room: Check when applicable.
- Hospitalized Overnight: Check when applicable.
- Take the following actions:

On Call Nurse (Telemed) Details
The On Call Nurse (Telemed) Details section will appear when On Call Nurse (Telemed) is selected as a severity.
Follow these steps to complete the On Call Nurse (Telemed) Details section. All entry fields are denoted are optional entry fields and are denoted in BLACK.
- Call Date: Enter the date the injured person called.
- On Call Service Name: Type the name of the person who took the call.
- Treatment Recommendation: Type the applicable recommendation.
- Take the following actions:

Body Parts Injured
The Body Parts Injured section is always available to enter one or more body parts injured during the incident.
Follow these steps to complete the Body Parts Injured section.
- Scroll down the screen to view the Body Parts Injured section
- Click the Add a Body Part button
- The body part row will appear. Make the following entries:
- Primary: Indicates primary injury. When multiple body parts are injured, you will be able to select which is the primary injury.
- Type: Select the type on injury from the Type drop down list.
Note: The Type column must be completed even though it is not denoted as required entry field - Body Part: Select from the body part Body Part drop down list.
Note: The Body Part column must be completed even though it is not denoted as a required entry field. - Left/Right: Only required when the body part selected has a left and right option. For example, hands.
- Repeat steps 1-6 to add additional body parts injured as necessary.

OSHA Compliance
The OSHA Compliance section is always available for completion. You will be able to enter whether the incident is considered recordable. All entry fields are denoted are optional entry fields and are denoted in BLACK.
Generally, an OSHA recordable injury or illness is one that requires medical treatment beyond first aid, causes death, days away from work, restricted work or transfer to another job, or loss of consciousness.
Follow these steps to complete the OHSA Compliance section.
- Scroll down the screen to view the OSHA Recordable section.
- OSHA Recordable: Check if you deem this to be an OSHA Recordable incident.
- Recordable Rationale: Type the applicable rationale.
- Completed by User: Select the user that completed the OSHA Compliance section.
- Completed On: Select the Date and Time.
- Take the following actions:

- When saved,
- A case number will be added to the OSHA Compliance section. The case number will appear on the OSHA Form 300 Log of Work Related Injuries and Illnesses and OSHA Form 301 Injury and Illness Incident Report.
- The OSHA Form 301 button at the top of the screen will become active for the Incident. Please refer to the OSHA Form 301: Injury and Illness incident Report article for more details for how to produce the report.
- Repeat the steps for remaining injured persons.

EDIT AN INJURY
Follow these steps to edit an existing injury on the Injuries tab.
- Search and locate the desired incident.
- Click on the Injuries tab.
- Click the Edit button (pencil icon) to the left of the desired injured person
- Take the following actions:

- Repeat Steps 3-4 to continue editing the incident.
DELETE AN INCIDENT
Follow these steps to delete an existing injury on the Injuries tab. Deleting the injury does not delete the user record or the user from the incident.
- Search and locate the desired incident.
- Click on the Injuries tab.
- Click the Delete button (trash bucket icon) to the left of the desired injured person.
- Click the Yes button on the Confirmation Screen.
- Click the Save button.
- You will be returned to the Injuries tab.
- Repeat Steps 3-6 to delete other injuries.
