INTRODUCTION
OSHA Form 300A: Summary of Work Related Injuries and Illnesses is a summary of the work-related injuries and illnesses that occurred at the workplace in the previous filing year.
Generally, an OSHA recordable injury or illness is one that requires medical treatment beyond first aid, causes death, days away from work, restricted work or transfer to another job, or loss of consciousness.
Form 300A is required to be submitted for establishments that:
Note: Certain states are exempt from filing the Form 300A but they are still required to post the form at each establishment.
- Have greater than 250 employees.
- Have between 20-249 employees AND are considered high risk industries.
Note: A high risk industry are companies with 20-249 employees and the first 2 to 4 digits of the establishment's NAICS Code. Please click on the OSHA High Risk Industry NAICS Codes to view the list.
Form 300A must be printed and posted for each establishment from February 1 through April 30 annually.
The IMS allows you to Submit Data to OSHA in 3 ways:
- File Electronically
- Upload a CSV File
- Enter Manually
All OSHA forms must be retained for five years.
SAMPLE REPORT
The sample report above includes a color key to indicate the record that data is pulling from to complete the report.
- Turquoise: Establishments Record
- Pink: Filing Wizard on establishments (you will enter the annual average number of employees and total hours worked when you file or print the report)
- Green: Manually enter the data in these fields
- Yellow: System Data - aggregated by incidents reported in the IMS

SUBMIT DATA: FILE ELECTRONICALLY
In order to submit this report, you must have a security role of Incident Manager Master or Incident Manager Admin.
ITA Token
Before you can upload your CSV file, you must create an OSHA Injury Tracking Application account. During this process, you will be asked to provide your ITA token. Instructions to retrieve your ITA token are listed below.
Follow these steps to file your report electronically:
- Click the Administration tab at the top of the screen.
- Click Incident Management, then Establishments on the left menu bar.
- Search and locate the establishment(s) you would like to submit the OSHA 300A for.
- Select the desired establishment(s). You may file one or more establishments at a time.
Note: You can select one or more establishments by using your CTRL + Left Mouse (Non-Sequential selection) SHIFT + LEFT Mouse (Sequential Selection). - Click the Filing Wizard button located at the top of the screen.
- Click the Submit 20XX data button.
Note: XX represents the current filing year. - Click the Next button.
- Click the File Electronically button.
- Click the Next button.
- Enter the Avg Employees and Total Hours Worked and repeat this step for all the establishments selected.
Note: If you have used the Rate Data feature in the IMS, these fields will automatically populate. By entering the Hours Worked Actual and the # of Employees for every month; the IMS automatically determines the average hours worked and # of employees for you and populates that number in these fields. - Click the Next button.
- Login to the OSHA site to retrieve your token.
- Click the OSHA Injury Tracking Application link.
- Login to the OSHA Injury Tracking Application site.
- Click the View Your ITA Token button located in the For Batch Data Transmission section of the Injury Tracking Application Home Page.
- Click the Copy Token button in the ITA Token section.
- Return to the Incident Management System.
- Paste the token in the ITA Token field.
- Click the Next button.
- Click the Submit button.
- You will be returned to the Establishments screen. The Form 300A Submitted column will change to a green check-mark and the current date will be populated.
SUBMIT DATA: UPLOAD A CSV FILE
In order to submit this report, you must have a security role of Incident Manager Master or Incident Manager Admin.

- Click the Administration tab at the top of the screen.
- Click Incident Management, then Establishments on the left menu bar.
- Search and locate the establishment(s) you would like to submit the OSHA 300A for.
- Select the desired establishment(s). You may file one or more establishments at a time.
Note: You can select one or more establishments by using your CTRL + Left Mouse (Non-Sequential selection) SHIFT + LEFT Mouse (Sequential Selection). - Click the Filing Wizard button located at the top of the screen.
- Click the Submit 20XX data button.
Note: XX represents the current filing year. - Click the Next button.
- Click the Upload CSV File button.
- Click the Next button.
- Enter the Avg Employees and Total Hours Worked and repeat this step for all the establishments selected.
Note: If you have used the Rate Data feature in the IMS, these fields will automatically populate. By entering the Hours Worked Actual and the # of Employees for every month; the IMS automatically determines the average hours worked and # of employees for you and populates that number in these fields. - Click the Next button.
- Enter the current date in the Filing Date field.
- Click the Next button.
- Click the Download button to generate the .csv file.
- Save the downloaded file to your desktop or network location.
- The screen will move forward to Your Submission has been recorded screen.
- Click the Click here to visit the OSHA site link and follow these steps:
- Login to the OSHA Injury Tracking Application site
- Click the Upload a Batch File button located in the For Batch Data Transmission section.
- Follow the instructions to upload your file.
- Return to the Incident Management System.
- Click the Done button.
- You will be returned to the Establishments screen. The Form 300A Submitted column will change to a green check-mark and the current date will be populated.
SUBMIT DATA: ENTER MANUALLY
If you choose not to submit data through the IMS to file electronically or upload a .csv file, you may want to use the enter manually function to update the Establishments screen to indicate you have filed the report.
Follow these steps to enter the data that you manually submitted Form 300A:
- Click the Administration tab at the top of the screen.
- Click Incident Management, then Establishments on the left menu bar.
- Search and locate the establishment(s) you would like to manually enter a submission date.
- Select the desired establishment(s). You may enter a date for one or more establishments at a time.
Note: You can select one or more establishments by using your CTRL + Left Mouse (Non-Sequential selection) SHIFT + LEFT Mouse (Sequential Selection). - Click the Filing Wizard button located at the top of the screen.
- Click the Submit 20XX data button.
Note: XX represents the current filing year. - Click the Next button.
- Click the Enter Manually button.
- Click the Next button.
- Enter the current date in the Filing Date field.
- Click the Next button.
- Click the Submit button.
- You will be returned to the Establishments screen. The Form 300A Submitted column will change to a green check-mark and the current date will be populated.
PRINT FORM
Follow these steps to print the form:
- Click the Administration tab at the top of the screen.
- Click Incident Management, then Establishments on the left menu bar.
- Search and locate the establishment(s) you would like to print the OSHA Form 300A.
- Select the desired establishment(s). You may print the form for one or more establishments at a time.
Note: You can select one or more establishments by using your CTRL + Left Mouse (Non-Sequential selection) SHIFT + LEFT Mouse (Sequential Selection). - Click the Filing Wizard button located at the top of the screen.
Note: DO NOT select the printer icon to the left of the Establishment name. That printer icon button prints OSHA Form 300: Log of Work Related Injuries and Illnesses NOT Form 300A: Summary of Work Related Injuries. - Enter the Year in the Select Filing Year field you would like to print the form.
- Click the Next button.
- Enter the Avg Employees and Total Hours Worked and repeat this step for all the establishments selected.
Note: If you have used the Rate Data feature in the IMS, these fields will automatically populate. By entering the Hours Worked Actual and the # of Employees for every month; the IMS automatically determines the average hours worked and # of employees for you and populates that number in these fields. - Click the Next button.
- Check the checkbox Record as a Posted Copy if you will be filing using this printed copy.
- Click the Download button.
- OSHA Form 300A will appear on the screen.
Note: use the Next Page button if you selected multiple establishments to view the form for each establishment. - Click the Print button and follow the onscreen prompts to print the form.

- Click the Close button.
- You will be returned to the Establishments screen. The Form 300A Submitted column will change to a green check-mark and the current date will be populated.
