INTRODUCTION
You can create a quiz to test users on various aspects of their jobs.
Quiz Features
- Create True/False and Multiple choice questions
- Add images or videos to questions
- Modify the feedback received when a user gets a correct/incorrect question for each question
Preferences
- Set a minimum passing score
- Limit the number of attempts per quiz
- Limit time per attempt
- Modify the feedback received when a user gets a correct/incorrect question for the entire quiz
- Randomize the questions and/or answers
- Determine the number of questions to display
- Set maximum attempts per question
- Choose if multiple choice answers should be lettered or numbered
Once created, quizzes become activities that can be assigned to your users and are completed by achieving a passing score.
The user attains a Completion Record when they launch and complete the quiz and receive a passing score. Then, you can run reports to see who has completed the assigned training.
CREATE A QUIZ
Follow these steps to create a quiz:
- Click the Administration tab at the top of the screen
- Click the Training Import and Creation link, then the Content Wizard link on the left menu bar
Screen 1: How do you want to add content?
- Click the Create button
- Click the Start button
Screen 2: What type of content do you want to create?
- Click the Quiz button
- Click the Next button
Screen 3: Please specify a save location
- Click the Select a Repository link or the Select button
Note: A repository is much like a folder on a network or local hard drive and is used to store training content imported in the LMS. Refer to the Files and Repositories Overview article for more information on Repositories.- Select the checkbox to the left of the desired repository
- Click the Select button (you will be returned to the specify import location screen)
- Click the Next button
Screen 4: Set Activity Properties
- Display Name: Enter the name of the quiz.
- Description: Optional field. Use to type a description of the activity. This field is visible to all users.
- Activity Duration: Enter the estimated time to complete the activity.
Note: Be as specific as possible. This field appears on the Learner's Training Tab and will inform the user how long it will take to complete the activity so they can budget their time accordingly. - Make Public: Choose to allow this item to be shared with other LMS Administrators in different parts of your organization so they can upload it to their location. Refer to the Sharing Files between Locations article to learn more about file sharing and making files public.
NO: Default. The file will not be shared
YES: The file will be shared - Click the Next button
- The Edit Quiz screen will appear. It is made up of two rows of buttons. The top button bar will include mostly inactive buttons that will become active as you begin to build the quiz. The bottom button bar presents the types of questions you can add to the quiz.

Follow the steps below to create the quiz:

Add Information Slide
An Information slide is OPTIONAL. It is useful when you would like to:
- Provide instructions to the user about completing the quiz.
- Upload a video view prior to taking the quiz. Then, have the user answer questions related to the video just viewed.
- Add an image to view prior to taking the quiz.
- Provide a link to a document to read prior to taking the quiz. Then, have the user answer questions related to the document just read.
- Provide a link to a website to refer to before answering questions.
Follow these steps to add information slide to a quiz:
- Click the Info button
- Slide Title: Type the slide title. For example, Quiz Overview or Quiz Instructions
- Slide Text: Type instructions that you would like the User to know about the quiz
Note: Use the button bar above the text box to format the text with bold, italic, increase or reduce the font size, add a bulleted or numbered list, or add a link to the content. - Add Media: Click the Add Media button to add a picture or vidoe. Refer to the Media Library: Add Remove Media from a Quiz, Tasklist or Survey section to learn how to add or remove media from the media library.
- LINK URL: Type a URL to a document or website.
Notes: (1) You can add a link to a file on stored on Google Drive, a SharePoint page, website, or intranet site. Web browsers will no longer allow you to link to a file on a network drive. (2) Keep in mind that if the link you provide is behind your firewall, the user will not be able to launch the link if accessing the LMS outside of your company’s network. For example, working remotely. - Click the Done button to save changes to the information field
- Refer to the Add True/False Questions, Add Single Answer Questions , and Add Multiple Choice Questions sections below as needed.

Add True/False Questions
Follow these steps to add true or false questions:
- Click the True/False button
- Slide Text: Type the True/False statement or question.
Note: Use the button bar to format the text with bold, italic, increase or reduce the font size, add a bulleted or numbered list, or add a link to the content. - Add Media: Click the Add Media button to add a picture or vidoe. Refer to the Media Library: Add Remove Media from a Quiz, Tasklist or Survey section to learn how to add or remove media from the media library.
- Answers: Select the correct response (either true or false).
- Feedback: Customize the feedback that a user will see when they select the correct or incorrect response to this statement or question.
Note: The default feedback will display Correct or Incorrect when the user selects the correct or incorrect response. Refer to the Set Quiz Properties section of this article to learn how to customize feedback for the entire quiz. - Click the Done button to save the question and return to the Edit Quiz screen.
- Repeat these steps until all true and false questions have been entered.
- Refer to the Add Information Slide, Add Single Answer Questions , or Add Multiple Choice Questions as needed.
- Refer to the Set Quiz Properties section of this article to set the Quiz Properties if you have completed entering questions and are ready to Save and Publish the quiz.
Note: It is recommended that you set the Properties of the Quiz before you Save and Publish the first time. If you set the properties after you save the quiz, you will be required to save the quiz again. - Click the Save button to save the quiz if you have completed entering questions and are ready to publish.

Add Single Answer Questions
A Single Answer Question is a mutliple choice question with only one correct answer.
Follow these steps to add a Single Answer question.
- Click the Single Answer button
- Slide Text: Type the question.
Note: Use the button bar to format the text with bold, italic, increase or reduce the font size, add a bulleted or numbered list, or add a link to the content. - Add Media: Click the Add Media button to add a picture or vidoe. Refer to the Media Library: Add Remove Media from a Quiz, Tasklist or Survey section to learn how to add or remove media from the media library.
- Answers: Complete the applicable answers in the Answers section
- Click the Add Answer button to add additional answers when there are more than three answers. You can have an unlimited number of answers.
- Check the box to the left of the answer to indicate the correct answer. You can only select one correct answer with this question type.
Notes: Once a correct answer is selected, the "Please select one or more correct answer" warning will disappear.
- Feedback: Customize the feedback that a user will see when they select the correct or incorrect response to this statement or question.
Note: The default feedback will display Correct or Incorrect when the user selects the correct or incorrect response. Refer to the Set Quiz Properties section of this article to learn how to customize feedback for the entire quiz. - Click the Done button to save the question and return to the Edit Quiz screen.
- Repeat these steps until all single answer questions have been entered.

Add Multiple Choice Questions
A Multiple Choice Question is a mutliple choice question with more than one correct answer.
Follow these steps to add a Multiple Choice question.
- Click the Multiple Choice button
- Slide Text: Type the question.
Note: Use the button bar to format the text with bold, italic, increase or reduce the font size, add a bulleted or numbered list, or add a link to the content. - Add Media: Click the Add Media button to add a picture or vidoe. - Answers: Complete the applicable answers in the Answers section
- Click the Add Answer button to add additional answers when there are more than three answers. You can have an unlimited number of answers.

Reorder Questions
From the Edit Quiz screen:
- Click on the question or information field to be re-ordered
- Holding the left mouse button, drag and drop the item to the new placement in the quiz
- Click the Save button to save the changes.
- Refer to the Publish the Quiz section of this article to publish the quiz and create/update the activity to assign to Users.
Copy Questions
From the Edit Quiz screen:
- Click the Copy button to the left of question or information field to be copied
- The item will be copied and added immediately following the question copied and appended with the word “Copy”
- Refer to the Edit Questions section of this article to edit the copied item.

Edit Questions
From the Edit Quiz screen:
- Click the Edit button to the left of question or information field to be edited
- Make any desired changes to the question and answer fields
- Click the Done button
- Click the Save button to save the quiz if once you have completed all edits
- Refer to the Publish the Quiz section of this article to publish the quiz and create the activity to assign to Users.

Delete Questions
From the Edit Quiz screen:
- Click the Delete button to the left of question or information field to be deleted.
- Click the Yes button on the Confirmation screen
- Click the Save button to save the quiz once you have completed all edits
- Refer to the Publish the Quiz section of this article to publish the quiz and create the activity to assign to Users

Set Quiz Properties
Properties allow you to set a passing score, randomize questions and answers, customize feedback, limit the attempts a user can answer a question and more.
From the Edit Quiz screen:
- Click the Properties button
- Refer to the Activity Tab | Quiz Tab | Questions Tab | Advanced Tab sections in this article below to learn about the available properties.
- Once all selections are made on all the tabs, click the OK button to save the changes. You will be returned to the Edit Quiz screen
- Click the Save button to save the quiz if you have completed entering questions and are ready to publish.
- Refer to the Publish the Quiz section of this article to learn how to publish the quiz.

Activity Tab
- Quiz Title: The name of the quiz defaults in this field. Modify the name of the quiz, if desired.
- Description: Optional Field. Use to describe the quiz.
- Activity Duration: Enter the estimated time to complete the quiz.
Note: Be as specific as possible. The duration will appear on the Training Tab when the quiz is assigned and will inform the user how long it will take to complete the activity.

Quiz Tab
- Passing Score: Enter the score (in terms of percentage) that your assigned learners must meet or exceed in order to pass the Quiz and generate a Completion Record.
Note: A quiz defaults to 80% passing score but you can change that to anything you like. - Limit Number of Attempts: Check the box if to limit the number of attempts the user can take this quiz. When checked, the # Attempts field will appear.
- # Attempts. Enter the number of attempts that the user has to pass the quiz.
- Limit Time Per Attempt. Check the box if you to set a time in minutes the user has to take the quiz. When checked, # Minutes field will appear.
- # Minutes. Enter the number of minutes allowed to complete the quiz.
- Feedback Type: Select one of the following feedback choices below to display when the learner answers question correctly or incorrectly:
- Do not show feedback: Nothing displays when the learner answers a question correctly or incorrectly.
- Show correct and incorrect feedback: (Default). Displays the “correct” when users answer the question correctly. Displays the “incorrect” when users answer the question incorrectly. You can change the default answers in the Correct and Incorrect text boxes. The text entered will display for ALL questions answered correctly or incorrectly when you do not chage the Feedback answers on the question.
- Show correct feedback and incorrect feedback with the answer: Displays the “correct” when users answer the question correctly. Displays the “incorrect” when users answer the question incorrectly AND displays the correct answer. You can change the default answers in the Correct and Incorrect text boxes. The text entered will display for ALL questions answered correctly or incorrectly when you do not change the Feedback answers on the question.

Questions Tab
- Display Order: Select one of the following two options:
- Specified Order: Displays questions in the order listed on the Create/Edit Quiz Screen
- Random Order: Randomizes questions so they present in a different order each time the quiz is launched.
Note: Do not randomize questions if you use an information field that you want to be fixed as the first item the user sees when they launch the quiz. Randomizing questions also randomizes the information fields.
- Attempts per question: The number of times the user can attempt to answer each question in the quiz.
- Answer Choice Enumeration: Select one of the following three options:
- None: Answers are not numbered or lettered.
- Numbered: Answer choices are numbered (1, 2, 3, 4…)
- Lettered: Answer choices are lettered (A, B, C, D…)
- Answer Order: Select one of the following two options:
- Specified Order: Displays answers in the order listed on the Edit Questions screen
- Random Order: Randomizes answers so they present in a different order each time the quiz is launched.
Note: Do not randomize answers if you frequently use “All of the above” or “None of the above” as an answer. Those answers may appear as the first item in the answer list.

Advanced Tab
- Make Public: Choose to allow this quiz to be shared with other LMS Administrators in different parts of your organization. Refer to the Sharing Files between Locations article to learn more about file sharing and making files public.
- NO: Default. The file will not be shared.
- YES: The file will be shared.
- Custom CSS: Please contact the Convergence Support Department at support@convergencetraining.com if you would like to further modify the look of the quiz.

PUBLISH THE QUIZ
Publishing the Quiz creates or updates the activity to be assigned to the user. You will not be able to assign a quiz until the quiz is published.
Notes: (1) If the Save and Publish buttons are inactive, it means that the most current version of the quiz has already been saved and published. (2) You do not have to publish the quiz until you are ready to assign the quiz. If you do not publish the quiz, it will not appear in your picklists. It stays in "draft" mode and you can continue to make changes until you are ready to publish and assign to users.
From the Edit Quiz screen:
- Click the Save button to save the quiz if you have not already done so.
Note: The Publish button will not become active until the Quiz has been saved. - Click the Publish button.
Review Settings Screen
- Review the settings and select one of two options:
- Cancel button: cancels publishing the quiz
- Publish button: click to continue publishing the quiz. If this is the first time you have published this quiz
- Click the OK button on the Attention Alert (Publish Complete) screen.
- You will be returned to the Edit Quiz screen.
- If you have published this quiz before, the Previous Completions screen will appear.
Previous Completions Screen
This screen appears when you have made a change to an existing quiz. It displays the number of users that have previously completed completed this quiz. Select one of two options:
- Publish and don’t change users’ completion status: this option keeps a user’s completion status intact and they will not be required to complete it again.
- Publish and set users back to incomplete: requires the user to recomplete the activity even if they completed it before.
- Publish Notes: Optional field that can be used to describe the changes made.
- Click the Cancel button to cancel putblishing
- Click the Publish button to continue publishing the quiz.
- Click the OK button on the Attention Alert (Publish Complete) screen
- You will be returned to the Edit Quiz screen
- Click the Close button to close the quiz

PREVIEW A QUIZ
Preview on the Edit Quiz Screen
While you are editing a quiz, follow these steps:
- Save the quiz prior to previewing
- Click the Preview button on the menu bar to preview the quiz.
Note: A warning will appear if you have not created any answers to a question or have not selected a correct answer to a question. This must be resolved prior to previewing the quiz. - Click the “X” button at the bottom of the screen to return to the Edit Quiz screen.
Preview After Publishing
Follow these steps to preview a quiz after the quiz has been published:
- Click the Administration tab at the top of the screen
- Click the Content Import and Creation link and then the Manage Quizzes link on the left menu bar
- Search and locate the quiz following the directions provided in Using Search article
- Click the Preview button to the left of the quiz. The quiz will open.
- Click the “X” button at the bottom of the screen to return to the Manage Quizzes screen

EDIT A QUIZ
Follow these steps to edit an existing quiz:
- Click the Administration tab at the top of the screen
- Click the Training Import and Creation link, then the Quizzes link on the left menu bar
- Click the Edit button to the left of the desired quiz.
- Click on the links below to locate instructions to make any desired changes to the quiz:

COPY A QUIZ
Follow these steps to copy a quiz:
- Follow the steps to Edit a Quiz
- Click the Save As button located on the Menu bar to create a new version of the quiz.
Note: Modifying or reordering a question/answer will not create a new version of the quiz. A new version is created when a change is made to the content of a question or information field. - Type the new Quiz title in the Quiz Title Field
- Click the Select a Repository link or the Select button
Note: A repository is much like a folder on a network or local hard drive and is used to store training content imported in the LMS. Refer to the Files and Repositories Overview article for more information on Repositories. - Select the checkbox to the left of the desired repository
- Click the Select button (you will be returned to the Save As screen)
- Click the Save button
- Click on the links below to locate instructions to make any desired changes to the quiz: Follow the steps below to:
- Add Information Slide | Add True/False Questions | Add Single Answer Questions | Add Multiple Choice Questions | Reorder Questions | Copy Questions | Edit Questions | Set Quiz Properties |
- Click the Save button when your edits are complete
- Follow the steps to Publish the Quiz
Note: The new version of the quiz will not be made available to users until the Publish button is selected. - Click the Close button.

RESTORE A PREVIOUS VERSION OF A QUIZ
The LMS stores previous versions of quizzes that you have created and allows you to restore any version of any quiz previously created.
Notes: (1) The Version column on the Manage Quizzes screen indicates the number of previous versions available. When the column reflects 1, there are no other previous versions to restore from. (2) Restoring a previous version will overwrite the current version and is NOT reversible.
Follow these steps to restore a previous version of a quiz:
- Click the Administration tab at the top of the screen
- Click the Training Import and Creation link, then the Quizzes link on the left menu bar
- Search and locate the quiz following the directions provided in the Using Search article
- Click the Edit button to the left of the desired quiz
- Click the Previous Versions button on the button bar to select desired quiz
Note: The Previous Versions button will not be active if there have not been any previous versions saved. - Click the button to the left of the version to be restored.
Note: This will overwrite the current version and is NOT reversible. - Click the Revert button
- Click the Yes button on the Confirm screen to replace the current version with the selected version.
- You will be returned the Edit Quizzes screen
- Click on the links below to locate instructions to make any desired changes to the quiz: Follow the steps below to:
- Add Information Slide | Add True/False Questions | Add Single Answer Questions | Add Multiple Choice Questions | Reorder Questions | Copy Questions | Edit Questions | Set Quiz Properties |
- Click the Save button if changes were made to the quiz
Note: The save button will not become active until a change has been made to the content of the quiz. - Follow the steps to Publish the Quiz
Note: The new version of the quiz will not be made available to users until the Publish button is selected. - Click the Close button.
DELETE A QUIZ
Deleting a quiz is generally discouraged as all user completion records associated with that quiz will also be deleted.
Note: Deactivate a quiz instead of delete. Refer to the Activities: Deactivate and Activate article for instructions to deactivate a quiz.
Follow these steps to delete a quiz:
- Click the Administration tab at the top of the screen
- Click the Training Import and Creation link, then the Quizzes link on the left menu bar
- Search and locate the quiz following the directions provided in Using Search article
- Click the Delete button to the left of the desired quiz name
Note: If the Delete button is inactive, you have not been assigned the Master administration role and will be unable to delete quizzes. - Click the Yes button on the Confirm Delete screen
- Click the OK button on the Delete Confirmation screen. The quiz will be deleted from the LMS.
Should a quiz be deleted in error, email support@convergencetraining.com. Support can generally restore the deleted quiz and restore any user completion records associated with the quiz.
